RESUMO Desde a publicação das últimas recomendações para o diagnóstico e tratamento da Demência Vascular pela Academia Brasileira de Neurologia em 2011, avanços significativos ocorreram na terminologia e critérios diagnósticos. O presente manuscrito é resultado do consenso entre especialistas indicados pelo Departamento Científico de Neurologia Cognitiva e do Envelhecimento da Academia Brasileira de Neurologia (2020-2022). O objetivo foi atualizar as recomendações práticas para a identificação, classificação e diagnóstico do Comprometimento Cognitivo Vascular (CCV). As buscas foram realizadas nas plataformas MEDLINE, Scopus, Scielo e LILACS. As recomendações buscam fornecer uma ampla revisão sobre o tema, então sintetizar as evidências para o diagnóstico do CCV não apenas para neurologistas, mas também para outros profissionais de saúde envolvidos na avaliação e nos cuidados ao paciente com CCV, considerando as diferentes realidades dos níveis de atenção à saúde (primário, secundário e terciário) no Brasil.

ABSTRACT Since the publication of the latest recommendations for the diagnosis and treatment of Vascular Dementia by the Brazilian Academy of Neurology in 2011, significant advances on the terminology and diagnostic criteria have been made. This manuscript is the result of a consensus among experts appointed by the Scientific Department of Cognitive Neurology and Aging of the Brazilian Academy of Neurology (2020-2022). We aimed to update practical recommendations for the identification, classification, and diagnosis of Vascular Cognitive Impairment (VCI). Searches were performed in the MEDLINE, Scopus, Scielo, and LILACS databases. This guideline provides a comprehensive review and then synthesizes the main practical guidelines for the diagnosis of VCI not only for neurologists but also for other professionals involved in the assessment and care of patients with VCI, considering the different levels of health care (primary, secondary and tertiary) in Brazil.

RESUMO Introdução: Anosognosia, i.e. a perda da consciência dos próprios sintomas, é um achado muito comum em pacientes com demência e está relacionada a sintomas neuropsiquiátricos e a pior prognóstico. Embora a doença por Corpos de Lewy (DCL) seja a segunda demência degenerativa mais comum, há pouca evidência sobre anosognosia nessa doença. Objetivos: Este artigo teve como objetivo revisar a evidência disponível sobre anosognosia em pacientes com DCL, incluindo sua prevalência em comparação a outras condições neurológicas, gravidade e correlações anatômicas. Métodos: Foram feitas buscas nos bancos de dados PubMed, Web of Knowledge e PsycINFO por artigos que avaliassem anosognosia na DCL. Um total de 243 estudos foi encontrado, mas apenas 6 foram incluídos nesta revisão. Resultados: Potenciais riscos de viés de seleção, comparação ou resultado foram encontrados em todos os estudos selecionados. A maior parte dos estudos utilizou questionários de memória preenchidos pelo próprio paciente e os comparou a resultados de instrumentos preenchidos por informantes ou à performance cognitiva em tarefas neuropsicológicas. Indivíduos com DCL têm pior consciencia de memória do que idosos saudáveis, mas os resultados tocantes à diferença de anosognosia entre DCL e doença de Alzheimer (DA) são inconsistentes entre estudos. A presença de achados patológicos e de neuroimagem de DA parece aumentar a prevalência de anosognosia entre pacientes com DCL. Conclusão: Anosognosia é uma manifestação comum da DCL, mas não é possível afirmar como sua prevalência e gravidade se comparam à DA. A coexistência de achados patológicos de DA parece influenciar a consciência de déficits de memória na DCL.

ABSTRACT Background: Anosognosia, i.e. lack of awareness of one’s own symptoms, is a very common finding in patients with dementia and is related to neuropsychiatric symptoms and worse prognosis. Although dementia with Lewy bodies (DLB) is the second most common form of degenerative dementia, literature on anosognosia in this disease is scarce. Objectives: This paper aimed to review the current evidence on anosognosia in patients with DLB, including its prevalence in comparison with other neurological conditions, its severity and anatomical correlations. Methods: Database searches were performed in PubMed, Web of Knowledge and PsycINFO for articles assessing anosognosia in DLB. A total of 243 studies were retrieved, but only six were included in the review. Results: Potential risk of selection, comparison or outcome biases were detected in relation to all the studies selected. Most of the studies used self-report memory questionnaires to assess cognitive complaints and compared their results to scores from informant-based instruments or to participants’ cognitive performance in neuropsychological tasks. Subjects with DLB had worse awareness regarding memory than healthy older controls, but the results concerning differences in anosognosia between DLB and Alzheimer’s disease (AD) patients were inconsistent across studies. Presence of AD pathology and neuroimaging biomarkers appeared to increase the prevalence of anosognosia in individuals with DLB. Conclusion: Anosognosia is a common manifestation of DLB, but it is not clear how its prevalence and severity compare with AD. Co-existence of AD pathology seems to play a role in memory deficit awareness in DLB.

RESUMO. Os sintomas neuropsiquiátricos que podem ocorrer de modo isolado ou acompanhando manifestações cognitivas e neurológicas são ocorrências frequentes em variadas disfunções ou lesões cerebrais devidas a condições vasculares, traumáticas, neurodegenerativas ou sistêmicas. Através da história, muitos dos nomes mais proeminentes contribuíram para a definição clínica dos sintomas e síndromes mentais reconhecidos atualmente. O presente artigo visa prover uma visão abrangente sobre o desenvolvimento, desde os tempos antigos até os presentes, de alguns dos conceitos e constructos amplamente reconhecidos sobre tais transtornos neuropsiquiátricos.

ABSTRACT. Neuropsychiatric symptoms, which may appear alone or combined with cognitive and neurological manifestations, are frequent in many brain dysfunctions or lesions due to vascular, traumatic, neurodegenerative, or systemic conditions. Throughout history, many of the most prominent names have contributed to the clinical definition of the currently recognized mental symptoms and syndromes. The present paper aims at providing a comprehensive overview of the development, from ancient to modern times, of some widely known concepts and constructs about such neuropsychiatric disorders.

Abstract Background Parents and teachers can be valuable sources of information for characterizing children’s ADHD-related impairments in different environments. However, evidence indicated that those categories of informants often provide conflicting responses in formal assessment scales, which may challenge diagnostic decisions regarding the condition. Objective: We aimed to investigate reliability rates between parents and teachers of children and adolescents with and without ADHD using SNAP IV. Methods 199 children and adolescents aged 6 to 17 years were evaluated for ADHD symptoms using parent-rated and teacher-rated SNAP IV scales. Intraclass correlation coefficients were analyzed for ADHD domains (inattention and hyperactivity/impulsivity), as well as for defiant-oppositional behavior. Results Reports from parents and teachers showed low reliability for all ADHD domains. Parents’ scores on the SNAP IV were higher than those of teachers. Parents and teachers provided highly discrepant responses concerning to the presence and severity of ADHD in children and adolescents, which might result from intrinsic aspects related to their daily functioning in different settings. Discussion Clinicians should consider those trends in parental and teachers’ responses when interpreting results from informant-based instruments for detecting ADHD.

RESUMO Sintomas neuropsiquiátricos na demência são prevalentes, pouco reconhecidos e ainda pouco estudados quanto aos aspectos fisiopatológicos. O mecanismo fisiopatológico, assim como o possível papel das lesões vasculares na gênese destes sintomas ainda são motivos de debate. Objetivo: descrever e comparar a prevalência e severidade dos sintomas entre a doença de Alzheimer (DA) e demência vascular (DV). Métodos: estudo transversal que incluiu 82 pacientes selecionados, divididos em dois grupos (DA × DV). Foram submetidos ao teste cognitivo de Cambridge (CAMCOG), teste do desenho do relógio (CLOX 1 e 2), inventário neuropsiquiátrico (INP) e avaliação clínica do estágio da demência (CDR). Avaliação dos índices de neuroimagem foram pelo escore de de Leon e Fazekas. Resultados: 90.8% dos pacientes apresentavam pelo menos um sintoma neuropsiquiátrico. Houve diferença estatísticas no CLOX e sintoma apatia entre DA e DV. Apatia e desinibição demonstraram ser mais prevalentes nos pacientes com maior carga vascular. Conclusão: Apatia e alteração na função executiva podem refletir danos vasculares nos circuitos subcorticais naqueles pacientes com demência.

ABSTRACT Neuropsychiatric symptoms (NPS) in dementia are prevalent, under-recognized and little studied regarding their pathophysiological aspects. The pathophysiological mechanism, as well as the possible role of vascular lesions in the genesis of these symptoms, are still matters of debate. Objective: to describe and compare the prevalence and severity of NPS in subjects with Alzheimer's disease (AD) and vascular dementia (VaD). Methods: a cross-sectional study involving 82 outpatients, divided into two groups (AD × VaD), was conducted. Patients were submitted to the Cambridge Cognitive Test (CAMCOG), the Clock Drawing Test (CLOX 1 and 2), the Neuropsychiatric Inventory (NPI) and the Clinical Dementia Rating (CDR) scale. Neuroimaging was scored using the de Leon and Fazekas scales. Results: 90.8% of the patients had at least one neuropsychiatric symptom. There were statistical differences on the CLOX test and in the apathy symptoms between AD and VaD groups. Apathy and disinhibition proved more prevalent in patients with higher vascular load. Conclusion: apathy and impaired executive function may reflect vascular damage in subcortical circuits in dementia patients.

The Mini-Mental State Examination (MMSE) is the most widely used instrument for the screening of older adults with suspected cognitive impairment; the MMSE has been translated and validated in numerous languages and countries. The cultural and sociodemographic characteristics of the sample can influence performance on the test; therefore, applying the MMSE in different populations usually requires adjustments of those variables. From this perspective, the present study aims to review the normative data for the MMSE in the Brazilian older population, including those adaptations to the original test. Database searches were performed in Medline, Web of Knowledge, Scielo and Pepsic for articles assessing healthy elderly Brazilian samples using the MMSE. Heterogeneity across and within the studies was analyzed. Of a total of 1,085 retrieved articles, 14 were included. Significant differences across studies were identified for the characteristics of the samples, the presence of alterations to the MMSE subtests and the presentation of the results. The risk of biases was relevant for all the studies. Considering the large methodological heterogeneity among studies, the generalization of the available normative data for the MMSE may not be appropriate for the general elderly Brazilian population.

RESUMO. Introdução: O Comprometimento Cognitivo Vascular Subcortical (CCVS) é um contínuo clínico de comprometimento cognitivo de causa vascular, que inclui o Comprometimento Cognitivo Leve Vascular (CCLV) e a Demência Vascular. Déficits em Função Executiva (FE) são marcantes no quadro, mas a melhor metodologia para avaliar essa função ainda necessita ser determinada. A evolução insidiosa e quase previsível do CCVS e o conceito atual multidimensional de FE sugerem que uma dissociação temporal de comprometimentos dos domínios da FE exista nos estágios iniciais do transtorno. Objetivo: Visou-se a revisar e analisar dados da literatura sobre o desempenho de sujeitos com CCLV nos testes mais usados de FE através de uma abordagem metanalítica. Métodos: Foram realizadas buscas no Medline, Web of Knowledge e PsycINFO, usando os seguintes termos: “comprometimento cognitivo vascular” OU “comprometimento cognitivo não demência vascular” OU “transtorno neurocognitivo leve vascular” E “função executiva” OU “disexecutiva”. Metanálises foram conduzidas para cada um dos testes, usando-se modelos de efeitos aleatórios. Resultados: A revisão sistemática demonstrou grande discrepância entre os resultados dos estudos incluídos. A metanálise evidenciou desempenhos piores no Teste de Trilhas parte B e no teste de cores do Stroop em sujeitos com CCLV em comparação com controles. Conclusão: Um contínuo de comprometimentos em FE no CCVS foi proposto. Déficits iniciais parecem ocorrer em flexibilidade cognitive e controle inibitório.

ABSTRACT. Background. Subcortical Vascular Cognitive Impairment (SVCI) is a clinical continuum of vascular-related cognitive impairment, including Vascular Mild Cognitive Impairment (VaMCI) and Vascular Dementia. Deficits in Executive Function (EF) are hallmarks of the disorder, but the best methods to assess this function have yet to be determined. The insidious and almost predictable course of SVCI and the multidimensional concept of EF suggest that a temporal dissociation of impairments in EF domains exists early in the disorder. Objective: This study aims to review and analyze data from the literature about performance of VaMCI patients on the most used EF tests through a meta-analytic approach. Methods: Medline, Web of Knowledge and PsycINFO were searched, using the terms: “vascular mild cognitive impairment” OR “vascular cognitive impairment no dementia” OR “vascular mild neurocognitive disorder” AND “dysexecutive” OR “executive function”. Meta-analyses were conducted for each of the selected tests, using random-effect models. Results: Systematic review showed major discrepancies among the results of the studies included. Meta-analyses evidenced poorer performance on the Trail-Making Test part B and the Stroop color test by VaMCI patients compared to controls. Conclusion: A continuum of EF impairments has been proposed in SVCI. Early deficits appear to occur in cognitive flexibility and inhibitory control.

RESUMO. A última década vem testemunhando avanços substanciais na aquisição de marcadores biológicos para o diagnóstico da doença cerebrovascular (DCV). Os métodos de neuroimagem avançados não apenas fornecem uma contribuição estratégica para o diagnóstico diferencial do comprometimento cognitivo vascular (VCI) e demência vascular (VaD), mas contribuem substancialmente na elucidação dos mecanismos fisiopatológicos relacionados à doença de vasos pequenos (SVD) e sua progressão clínica. Objetivo: Nesta revisão, métodos de imagem estruturais e metabólicos foram descritos e sua importância diagnóstica analisada, particularmente na investigação da CVD relacionada ao envelhecimento. Métodos: uma pesquisa eletrônica de janeiro de 2010 a 2017 foi realizada através do PubMed/MEDLINE, do Instituto de Informação Científica Web of Knowledge e da EMBASE. Resultados: O emprego de estudos de multimodalidade plenamente funcional com Ressonância Magnética (MR)/ Tomografia por Emissão de Pósitrons (PET) representa uma janela para a caracterização clínica mais detalhada da VCI-VaD; com relação à neuroimagem estrutural, a ressonância magnética em 3,0 T vem permitindo varreduras com maior resolução e matrizes de imagem mais elevadas, cortes mais delgados e maior detalhamento anatômico das lesões vasculares. Conclusão: Embora a importância da maior parte dessas técnicas no cenário clínico aguarde reconhecimento, há uma grande expectativa de que o seu uso favoreça intervenções terapêuticas progressivamente mais precoces e refinadas para o gerenciamento efetivo do VCI-VaD.

ABSTRACT. The last decade has witnessed substantial progress in acquiring diagnostic biomarkers for the diagnostic workup of cerebrovascular disease (CVD). Advanced neuroimaging methods not only provide a strategic contribution for the differential diagnosis of vascular dementia (VaD) and vascular cognitive impairment (VCI), but also help elucidate the pathophysiological mechanisms ultimately leading to small vessel disease (SVD) throughout its course. Objective: In this review, the novel imaging methods, both structural and metabolic, were summarized and their impact on the diagnostic workup of age-related CVD was analysed. Methods: An electronic search between January 2010 and 2017 was carried out on PubMed/MEDLINE, Institute for Scientific Information Web of Knowledge and EMBASE. Results: The use of full functional multimodality in simultaneous Magnetic Resonance (MR)/Positron emission tomography (PET) may potentially improve the clinical characterization of VCI-VaD; for structural imaging, MRI at 3.0 T enables higher-resolution scanning with greater imaging matrices, thinner slices and more detail on the anatomical structure of vascular lesions. Conclusion: Although the importance of most of these techniques in the clinical setting has yet to be recognized, there is great expectancy in achieving earlier and more refined therapeutic interventions for the effective management of VCI-VaD.

RESUMO Infartos talâmicos bilaterais são em geral ocasionados por oclusão da "Artéria de Percheron" (AdP). A síndrome talamopeduncular está entre as apresentações clínicas mais comuns da oclusão da AdP. Um homem de 59 anos apresentou rebaixamento abrupto do nível de consciência. Após algumas semanas, ao recobrar a consciência, apresentava anormalidades oculomotoras, marcha atáxica, distonia cervical e alterações cognitivas e comportamentais. A imagem por ressonância magnética evidenciou infartos talâmico, subtalâmico, mamilar e mesencefálico. O quadro clínico foi sugestivo de síndrome talamopeduncular. Além da presença de comprometimento cognitivo e transtornos de comportamento, estava presente distonia cervical, que pode resultar da interrupção das interconexões entre gânglios da base, tálamo, subtálamo, mesencéfalo e cerebelo.

ABSTRACT Bilateral thalamic infarctions are usually caused by occlusion of the "Artery of Percheron" (AoP). Thalamopeduncular syndrome is among the most common presentations of AoP occlusion. A 59-year-old male presented abrupt decreased level of consciousness. After several weeks, on regaining consciousness, he exhibited oculomotor abnormalities, ataxic gait, cervical dystonia, and cognitive and behavioral changes. Magnetic resonance imaging disclosed thalamic, subthalamic, mammillary and midbrain infarction. Clinical features suggestive of bilateral thalamopeduncular syndrome were identified. Besides the presence of cognitive impairment and behavioral symptoms, cervical dystonia was evident, possibly resulting from interruption of the interconnections among basal ganglia, thalamus, subthalamus, midbrain and cerebellum.

RESUMO O reconhecimento precoce da Doença Cerebrovascular (DCV) pode permitir o controle de fatores de risco e a prevenção de demência. Contudo, critérios operacionais em seus estágios sintomáticos não-demenciais apresentam problemas, já que critérios atuais requerem a presença de extensa doença isquêmica subcortical. OBJETIVO Identificar o perfil de neuroimagem do Comprometimento Cognitivo Leve Vascular (CCLV), o impacto destes aspectos sobre a cognição e os testes neuropsicológicos que distinguem CCLV de outros grupos. MÉTODOS Foram realizadas buscas no Scopus, ISI e PsycINFO, usando a estratégia: "vascular mild cognitive impairment" OR "vascular cognitive impairment no dementia" OR "vascular cognitive impairment not demented" OR "subcortical mild cognitive impairment". RESULTADOS De 249 artigos, 20 foram selecionados. 10 destes incluíram apenas pacientes com hiperintensidades de substância branca (HSB) graves, enquanto 10 outros admitiram pacientes com HSB moderadas-a-graves. Ambos os grupos apresentaram desempenho pobre em tarefas de Função Executiva (FE) em comparação com controles normais e outras categorias diagnósticas. Dentre os testes de FE, aqueles que avaliam FE "complexas" diferiram consistentemente CCLV de outros grupos, independentemente da gravidade de HSB. Sujeitos com CCLV e HSB graves ou moderadas-a-graves apresentaram dificuldades cognitivas quando comparados aos demais grupos. Testes que avaliam FE "complexa" foram os mais úteis na diferenciação destes pacientes dos outros grupos. CONCLUSÃO A ocorrência de VaMCI pode estar associada à presença de HSB moderadas; a detecção precoce do dano vascular permitiria a adoção de medidas terapêuticas com tamanhos de efeito significativos.

ABSTRACT The recognition of Cerebrovascular Disease (CVD) at earlier clinical stages may favor the control of vascular risk factors and prevention of dementia. However, operational criteria for symptomatic phases at non-dementia stages are often difficult, as the current criteria normally require the evidence of extensive subcortical disease. OBJECTIVE To identify the neuroimaging profile of Vascular Mild Cognitive Impairment (VaMCI), the impact of those aspects over cognition and the neuropsychological tests that distinguished VaMCI from other groups. METHODS Searches were performed in Scopus, ISI and PsycINFO, using the following key terms: "vascular mild cognitive impairment" OR "vascular cognitive impairment no dementia" OR "vascular cognitive impairment not demented" OR "subcortical mild cognitive impairment". RESULTS Of 249 papers, 20 studies were selected. Ten of those included only patients with severe White Matter Hyperintensities (WMH), whereas 10 others admitted subjects with moderate-to-severe WMH. Both groups showed poor performances in Executive Function (EF) tasks in comparison to normal controls and other diagnostic groups. Among EF tests, those assessing "complex" EF abilities consistently distinguished VaMCI from other groups, regardless of the severity of WMH. VaMCI subjects with severe or moderate-to-severe WMH showed cognitive deficits in comparison with other groups. "Complex" EF tests were the most useful in differentiating those patients from the other groups. CONCLUSION The occurrence of VaMCI may be associated with the presence of CVD at moderate levels; the detection of vascular damage at earlier stages may allow the adoption of therapeutic actions with significant effect-sizes.

ABSTRACT Arteriopatia Cerebral Autossômica Dominante com Infartos Subcorticais e Leucoencefalopatia (CADASIL) é uma desordem hereditária que acomete a vasculatura cerebral devido a mutações no gene do NOTCH 3. O diagnóstico pode ser estabelecido através de testes genéticos para a detecção das mutações e/ou de biópsia de pele. É relatado um caso da desordem em paciente feminino, que apresentou ataques isquêmicos transitórios recorrentes, evoluindo para um quadro progressivo de demência subcortical. A neuroimagem mostrou extensa leucoaraiose e infartos lacunares. A análise genética para NOTCH 3 foi confirmatória. O exame ultraestrutural de biópsia de pele, inicialmente negativa, confirmou a presença de alterações características (presença de material granular osmiofílico [GOM]), após a análise de novos cortes do mesmo material. Os presentes achados indicam que achados negativos de exames ultraestruturais de biópsia não deveriam excluir o diagnóstico da doença e que análise adicional da amostra pode ser necessária para detectar a presença de GOM.

ABSTRACT Cerebral Autosomal Dominant Arteriopathy with Subcortical Infarcts and Leukoencephalopathy (CADASIL) is a hereditary disorder which affects the cerebral vasculature due to mutations in the NOTCH 3 gene. The diagnosis may be established through genetic testing for detection of these mutations and/or by skin biopsy. We report a case of the disorder in a female patient, who presented recurrent transient ischemic attacks that evolved to progressive subcortical dementia. Neuroimaging disclosed extensive leukoaraiosis and lacunar infarcts. The genetic analysis for NOTCH 3 was confirmatory. The ultrastructural examination of the skin biopsy sample, initially negative, confirmed the presence of characteristic changes (presence of granular osmiophilic material inclusions [GOM]), after the analysis of new sections of the same specimen. The present findings indicate that negative findings on ultrastructural examinations of biopsy should not exclude the diagnosis of the disease and that further analyses of the sample may be necessary to detect the presence of GOM.

Sintomas Neuropsiquiátricos ou Sintomas de Comportamento e Psicológicos da Demência (SCPD) são comuns e aparecem em algum ponto no curso da enfermidade, mediado por doenças cerebrovasculares e processos neurodegenerativos. Poucos estudos compararam os perfis dos SCPD no Comprometimento Cognitivo Vascular (CCV) de diferentes subtipos (subcortical ou córtico-subcortical) e diferentes estágios clínicos (Comprometimento Cognitivo Vascular Não Demência [CCV-ND] e Demência Vascular [DV]). Objetivo: Revisar os SCPD associados aos diferentes subtipos e estágios do CCV, com o Inventário Neuropsiquiátrico (INP). Métodos: Medline, Scielo e Lilacs foram pesquisados de janeiro de 2.000 até dezembro de 2.014, com as palavras chaves: "BPSD AND Vascular Dementia", "NPI AND Vascular Dementia" e "NPI AND VCI". Uma análise quantitativa foi feita nos estudos que avaliaram sintomas comportamentais no CCV através do INP. Resultados: Um total de 82 estudos foram identificados e 13 estudos foram considerados elegíveis e incluídos. Destes artigos, 4 compararam SCPD entre DV Córtico-Subcortical (DVCS) e DV Subcortical (DVS), 3 artigos compararam DVS e CCVND, 1 artigo analisou diferenças entre DVCS e CCVND, e 5 estudos avaliaram os SCPD em DVCS. Tanto a DVCS quanto a DVS associaram-se principalmente a Apatia e Depressão. O CCVND pode apresentar menos sintomas comportamentais que a DV. Conclusão: O perfil dos SCPD difere entre diferentes estágios do CCV. O entendimento dos SCPD mais prevalentes nos subtipos do CCV poderia ser útil para facilitar o diagnóstico precoce e o manejo desses sintomas.

Neuropsychiatric symptoms or Behavioral and Psychological Symptoms of Dementia (BPSD) are common and invariably appear at some point during the course of the disease, mediated both by cerebrovascular disease and neurodegenerative processes. Few studies have compared the profiles of BPSD in Vascular Cognitive Impairment (VCI) of different subtypes (subcortical or cortical) and clinical stages (Vascular Cognitive Impairment No Dementia [VaCIND] and Vascular Dementia [VaD]). Objective: To review the BPSD associated with different subtypes and stages of VCI using the Neuropsychiatric Inventory (NPI). Methods: Medline, Scielo and Lilacs databases were searched for the period January 2000 to December 2014, with the key words: "BPSD AND Vascular Dementia, "NPI AND Vascular Dementia" and "NPI AND VCI. Qualitative analysis was performed on studies evaluating BPSD in VCI, using the Neuropsychiatric Inventory (NPI). Results: A total of 82 studies were retrieved of which 13 were eligible and thus included. Among the articles selected, 4 compared BPSD in Subcortical Vascular Dementia (SVaD) versus Cortical-Subcortical Vascular Dementia (CSVaD), 3 involved comparisons between SVaD and VaCIND, 1 study analyzed differences between CSVaD and VaCIND, while 5 studies assessed BPSD in CSVaD. Subcortical and Cortical-Subcortical VaD were associated predominantly with Apathy and Depression. VaCIND may present fewer behavioral symptoms than VaD. Conclusion: The profile of BPSD differs for different stages of VCI. Determining the most prevalent BPSD in VCI subtypes might be helpful for improving early diagnosis and management of these symptoms.

Comprometimento cognitivo leve vascular (CCLV) representa um estágio sintomático precoce do comprometimento cognitivo vascular e associa-se à disfunção fronto-executiva.MétodosVinte e seis indivíduos (idade: 73,11±7,90 anos; 65,4% mulheres; escolaridade: 9,84±3,61 anos) foram selecionados por meio de avaliação cognitiva e neuroimagem. Os dados clínicos e de neuroimagem do grupo CCLV (n=15) foram comparados com controles normais (CN; n=11) e correlacionados com a escala de Fazekas.ResultadosCCLV apresentaram piores desempenhos que CN no Trail-Making Test (TMT) B, erros no TMT B, diferença TMT B-A e pontuação final do Cambridge Cognitive Examination (CAMCOG). Verificaram-se correlações entre escala de Fazekas e desempenhos no TMT B (tempo total e erros), diferença TMT B-A e a pontuação final do CAMCOG.ConclusãoA extensão das hiper-intensidades de substância branca, no grupo CCLV, correlacionou-se com pior desempenho cognitivo global e com comprometimento em um grupo de funções fronto-executivas, como velocidade e alternância cognitiva e controle inibitório.

Vascular mild cognitive impairment (VaMCI) represents an early symptomatic stage of vascular cognitive impairment and might be associated to fronto-executive dysfunction.MethodsTwenty-six individuals (age: 73.11±7.90 years; 65.4% female; schooling: 9.84±3.61 years) were selected through neuropsychological assessment and neuroimaging. Clinical and neuroimaging data of VaMCI individuals (n=15) were compared to normal controls (NC, n=11) and correlated with Fazekas scale.ResultsVaMCI performed significantly worse than NC in Trail-Making Test (TMT) B, errors in TMT B, difference TMT B-A and Cambridge Cognitive Examination (CAMCOG) final scores. Correlations were found among scores in modified Fazekas scale and performances in TMT B (time to complete and errors), difference TMT B-A and CAMCOG total score.ConclusionExtension of white matter hyperintensities might be correlated to poorer global cognition and impairments in a set of fronto-executive functions, such as cognitive speed, set shifting and inhibitory control in VaMCI.

RESUMO Objetivo: Comprometimento Cognitivo Leve Vascular (CCLV) não-amnésico disexecutivo pode representar a fase pré-clínica da Demência Vascular (DV). Este estudo tem por objetivo descrever os aspectos clínicos, cognitivos e de neuroimagem do CCLV, além de avaliar padrões de progressão para demência. Métodos: Foram realizadas buscas nas bases de dados ISI Web of Knowledge, PubMed e Lilacs, usando os termos "comprometimento cognitivo leve" e "função executiva". Ao todo, 944 artigos foram obtidos. Resultados: Pacientes com CCLV apresentaram piores desempenhos em tarefas fronto-executivas, histórico de acidentes vasculares encefálicos, presença de hiperintensidades da substância branca periventriculares e profundas, e mais, sinais extrapiramidais e sintomas comportamentais. A disfunção executiva parece se relacionar à desconexão de circuitos fronto-parieto-subcorticais. A progressão para demência relacionou-se a déficits em linguagem, função executiva e atenção sustentada à primeira avaliação, e a hiperintensidades na substância branca periventricular extensas. Discussão: O CCLV cursa com comprometimento cognitivo em domínios não-memória e alterações da substância branca subcortical à imagem por RM alinhadas com os achados característicos da DV.

ABSTRACT Objective: Non-amnestic dysexecutive Vascular Mild Cognitive Impairment (VaMCI) may represent preclinical Vascular Dementia (VaD). The aim of this study was to summarize the clinical, neuropsychological and neuroimaging aspects of VaMCI; and to assess its patterns of progression to dementia. Methods: Searches were made in the ISI Web of Knowledge, PubMed and Lilacs databases, using the terms "mild cognitive impairment" and "executive function". Altogether, 944 articles were retrieved. Results: VaMCI cases had poorer performances on fronto-executive tasks, a higher prevalence of stroke, presence of periventricular and profound white matter hyperintensities on MRI images, as well as more extrapyramidal signs and behavioral symptoms. Executive dysfunction might be associated with disconnection of fronto-parietal-subcortical circuits. Progression to dementia was associated with baseline deficits in executive function, in simple sustained attention and language, and large periventricular WMH. Discussion: VaMCI develops with impairment in non-memory domains and subcortical white matter changes on MRI images, which are consistent with clinical and neuroimaging findings in VaD.

Resumo Introdução: Sintomas neuropsiquiátricos são comuns em pacientes com déficits cognitivos associados a processos neurodegenerativos e à doença cerebrovascular. Estudos apontaram que os Sintomas Comportamentais e Psicológicos da Demência (SCPD) podem ter relação com a gravidade do declínio cognitivo. Até o presente, o impacto da associação entre hiperintensidades da substância branca (HSB) e atrofia hipocampal (AH) sobre esses sintomas foi pouco explorado na literatura. Objetivo: Este estudo transversal objetiva descrever as características clínicas de uma amostra com extensas HSB, examinando a influência de diferentes graus de AH sobre os aspectos cognitivos, comportamentais e funcionais. Métodos: Cinquenta idosos atendidos em ambulatório (idade média: 76.86±8.70 anos; 58% mulheres; escolaridade média: 7.44±4.69 anos), com extensas HSB (escala modificada de Fazekas=3) e diferentes graus de AH (de acordo com a escala de de Leon) foram submetidos à avaliação cognitiva, funcional e comportamental. Resultados: Pacientes com AH leve-moderada a grave apresentaram piores pontuações no Mini-Exame do Estado Mental, Avaliação Cognitiva de Cambridge, Estadiamento Clínico da Demência e Questionário de Atividades Funcionais de Pfeffer, em comparação com indivíduos com AH ausente ou questionável. Apetite/Comportamento Alimentar foi o único grupo de sintomas neuropsiquiátricos associados à presença de AH em indivíduos com Comprometimento Cognitivo Vascular. Discussão: Embora a AH pareça apresentar impacto sobre desempenho cognitivo e funcional, parece apresentar pouca influência sobre sintomas comportamentais em pacientes com extensas HSB.

ABSTRACT Background: Neuropsychiatric symptoms are common in patients with cognitive impairments, mediated by both neurodegenerative processes and cerebrovascular disease. Previous studies have reported that Behavioral and Psychological Symptoms of Dementia (BPSD) might correlate with severity of cognitive decline. Thus far, the impact of the association between white-matter hyperintensities (WHM) and hippocampal atrophy (HA) on the incidence of these symptoms has been less studied. Objective: This cross-sectional study aimed to describe the clinical profile of a sample with large extensions of WMH, examining the association between different degrees of HA and cognitive, functional, and behavioral status. Methods: Fifty outpatients (mean age: 76.86±8.70 years; 58% female; mean schooling: 7.44±4.69 years) with large extensions of WMH (modified-Fazekas scale=3) on MRI and different degrees of hippocampal atrophy (according to de Leon Score) underwent cognitive, functional, and behavioral assessments. Results: Patients with mild-moderate to severe HA had worse performance on the Mini-Mental State Examination, Cambridge Cognitive Examination, Clinical Dementia Rating and Pfeffer's Functional Activities Questionnaire, compared to the group with none or questionable HA. Appetite/Eating Behavior was the only cluster of neuropsychiatric symptoms associated with presence of HA in Vascular Cognitive Impairment patients. Discussion: Although HA may exhibit distinct impact on cognitive performance and functional status, it appears to have little effect on behavioral symptoms in patients with high severity WMH.
